Submit Rare Plant Data

Thank you for your assistance in helping us track rare plant species. The form below is for the submission of rare vascular and nonvascular plants and rare lichens. Please fill in the following fields to the best of your abilities. We recognize that some information will be left blank.

  • Observer Information

  • Enter the name of the primary observer / field crew leader.
  • Enter the affiliation (if any) of the primary observer / field crew leader. If your affiliation is not included in the list, please select other and type your affiliation in the field that appears below.
  • Enter the address of the primary observer, field crew leader, organization, or affiliate office.
  • Enter the phone number for the primary observer / field crew leader.
  • Enter the email address of the primary observer / field crew leader.
  • Collection Information

  • Enter the scientific name of the rare plant taxon that you observed. Please do not abbreviate (except for the labels "ssp." and "var." if applicable).
  • Enter the date that you observed / collected the rare plant.

  • Enter the collection number (if any assigned) for the collected specimen in whatever format you prefer to use.
  • Did you send the Alaska Natural Heritage Program (or will you be sending us) the specimen you collected so that we can confirm the identification?
  • What would you like us to do with your specimen after we have verified its identification?
  • Where is the specimen deposited or going to be deposited? If you select "other," then please type the name or abbreviation of the herbarium in the field that appears below.

  • Locality Information

  • Describe the location or give instructions to find the occurrence again.
  • Specify whether you would like to submit the location as a point or a polygon. If you submit as a polygon, please attach a GPS track file, .kml or .shp file, map, or sketch in the file attachment section above.

  • Enter the latitude in decimal degrees.
  • Enter the longitude in decimal degrees.
  • Enter the datum here. If the datum you use is not listed here, select "other" and type the datum in the field that appears below.

  • Enter the elevation for the occurrence. If the occurrence is spread across a wide range of elevations, please enter the elevation at which the highest density of individuals occur or the average elevation.

  • Population Information

  • Provide the population size category. If there were fewer than 50 individuals, please provide the actual count in the field below.
  • Please include any observations relating to population size, health, vitality, trends, etc.
  • Habitat Information / Associated Species

  • Provide a detailed description of the habitat in which you observed the rare plant (alpine, forest, foothills, etc.).
  • Provide a measure of slope in degrees.
    Please enter a number from 0 to 90.
  • Describe the plant community in which the rare plant population occurred.
  • Provide a description of any other habitat data such as soil type, light, or soil moisture
  • List any associated species or vegetation communities observed in the vicinity of the rare plant occurrence.
